Thank you for purchasing one or more tickets to the North County "Looking Forward" Awards Dinner & Fundraiser.
Now we need to some information from you (if we haven't gotten it from you already).
1. Please send us your name and dinner selection and your guests names (if any) and their dinner selections.
The main course choices are:
- Pesto and Mozzarella Chicken (rolled in chicken breast, lightly breaded)
- Filet of Salmon (Lemon Dill Beurre Blanc)
- Butternut Squash Ravioli (Basil Marinara)
All meals are served with a salad, a main course, garlic mashed potatoes, mixed vegetables and dessert.

2. If you are attending to honor a particular person or organization, or if there is anyone else you would like to sit with, please let us know. We will do our best to accommodate you.
